Respondent’s license to practice allopathic medicine in the State of Arizona is summarily restricted. Respondent is prohibited from prescribing, administering or dispensing controlled substances or weight loss medication until he applies to the Board and receives permission to do so.

Physician’s practice is limited in that he shall not practice medicine in the State of Arizona and is prohibited from prescribing any form of treatment including prescription medications until Physician applies to the Board and receives permission to do so. Effective 4.16.19

Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-2501 (13) until he applies to the Board’s Executive Director and receives its affirmative permission to do so.Respondent may not apply for relief from this Practice Limitation until he has completed any current treatment program, has been found safe to perform health care tasks, and the Board is in receipt of any discharge report and recommendations for aftercare. This Order supersedes previous Interim Practice Limitation.
