Application fees are non-refundable.
To obtain an Arizona Fingerprint Clearance Card, you may apply online at https://psp.azdps.gov/services/fccFormTriage.
For questions about the process, you may contact the Department of Public Safety (DPS) by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.
PHARMACY TECHNICIAN TRAINEE
Please note: A.A.C. R4-23-1103(C)(5) limits the time to 36 months for which a licensed pharmacy technician trainee may practice as a licensed pharmacy technician trainee. The Board interprets the intent of this limitation to preclude a licensed pharmacy technician trainee from applying to become a registered pharmacy technician trainee after their license has expired, been surrendered or revoked. Please refer to Substantive Policy Statement No. SPS 2023-01.
Pharmacy Technician Trainee Registration applicants must submit :
- $25.00 application fee (this fee is non-refundable)
- Proof of legal United States residency and birth date
- Government-issued photo ID (If the document you submit to prove your legal residency does not contain a photograph).
- Documentation of any name changes.

PHARMACY TECHNICIAN
Please note: If you held a pharmacy technician license with this Board, and that license has expired, please email licensing@azpharmacy.gov about renewing or reinstating your license.
Pharmacy Technician applicants must submit :
- $82.00 License application fee (this fee is non-refundable)
- Proof of legal United States residency and birth date.
- Government-issued photo ID. (If the document you submit to prove your legal residency does not contain a photograph).
- Proof of high school graduation or its equivalent.
- Proof of passing the PTCB or ExCPT exam.
- A verification of licensure or certification, if applicable.
- A copy of a valid Arizona fingerprint clearance card.
- Documentation of any name changes.
- Court documentation, if applicable.
- Disciplinary orders, if applicable.
- License denial orders, if applicable.

Renew / Reinstate License
Pharmacy technicians may log in to their online profile to renew their license. Create or login to your profile.
Pharmacy technician trainee licenses / registrations are not eligible for renewal.
Please note: Fingerprint clearance cards are not required for renewal of pharmacy technician licenses.
You must submit a reinstatement application if:
1. Your Arizona pharmacy technician license has been delinquent for five (5) or more consecutive years; or
2. Your Arizona pharmacy technician license was voluntarily surrendered by consent agreement or revoked by Order of the Board.
Verify License
The Arizona State Board of Pharmacy no longer provides hardcopy verifications of licensure; however, you may obtain primary source verification of a license here. Please note: primary source verification is not a license.
