Third Party Logistics Provider (3PL) Permit Information
Apply For a Permit
To obtain an Arizona Fingerprint Clearance Card, you may apply online at https://psp.azdps.gov/services/fccFormTriage. For questions about the process, you may contact the Department of Public Safety (DPS) by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.
- Plans or construction drawings showing facility size, designated area for storage and quarantine, and security for the proposed business;
- Documentation of compliance with local zoning laws (Lease or Zoning Statement);
- Designated Representative's resume.
- A copy of Designated Representative's valid Arizona fingerprint clearance card.
- A list of officers or partners, if applicable.
- A list of similar businesses, if applicable.
- Disciplinary Orders, if applicable.
- Denial letters, if applicable.
- Application fee (this fee is nonrefundable)

Renew Permit
3PL permit holders will need to log in through their profile to renew their permit online. Creating a profile is free.

3PL permit holders may also submit a hard copy renewal form with a check or money order by postal mail or at the Board office.
Submitting a hard copy renewal form can take 3-4 weeks for processing.

The Arizona State Board of Pharmacy no longer provides hardcopy verifications of licensure, however we are still able to offer this service via our website.
