Respondent’s practice is restricted in that she shall not practice medicine in the state of Arizona and is prohibited from prescribing any form of treatment including prescription medications until the Respondent applies to the Board and receives permission to do so. Effective 4.16.19

Interim Practice Restriction – Respondent shall not prescribe controlled substances in the State of Arizona and shall not have access to controlled substances in the workplace during the period of this Order.

Respondent is restricted in that he is prohibited from prescribing controlled substances until he has completed the required CME, enters into an Agreement with a Board-approved monitor to conduct chart reviews and provides satisfactory proof of compliance with these requirements. Effective 2-5-19.

Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until the Respondent applies to the Executive Director and receives permission to do so.
