Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until Respondent applies to the Executive Director and receives permission to do so.

Interim Practice Limitation- (Non- Disciplinary). Physician is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401 (22) until he applies to the Board and receives its affirmative permission to do.

Respondent’s license to practice allopathic medicine in the State of Arizona is summarily suspended. Respondent is prohibited from practicing medicine in the State of Arizona and is prohibited from prescribing any form of treatment including prescription medications or injections of any kind.
