Respondent's license No. 03-766 is placed on probation for twelve months, effective from the Board's dated signature below. During the term of probation, Respondent must comply with the following conditions, and his compliance will be monitored: During the time of probation Respondent's license is restricted and he shall not prescribe any schedule II controlled substances.
