Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until Respondent applies to the Executive Director and receives permission to do so.

Respondent is restricted in that he is prohibited from prescribing controlled substances until he has completed the required CME, enters into an Agreement with a Board-approved monitor to conduct chart reviews and provides satisfactory proof of compliance with these requirements. Effective 2-5-19.

Interim Practice Restriction – Respondent is prohibited from examining or treating female patients in all settings, including but not limited to office, hospital, and clinic until Respondent applies to the Executive Director and receives permission to do so.

Interim Practice Restriction – Respondent’s practice is restricted in that he may administer and prescribe controlled substance as part of the management of surgical patients in the operating and in preoperative areas of recovery areas at a hospital or surgical center. He shall not otherwise prescribe controlled substances and shall not prescribe to patients for use outside of hospital settings.
