Amended Consent Agreement for Civil Penalty. License issued upon payment of $250.00 civil penalty.

Decision and Order. License Revoked. Revocation stayed for 90 days, suspension and $250.00 civil penalty imposed.

Consent Agreement for Civil Penalty. The consent agreement includes a $1,200.00 civil penalty; an inspection within 90 days; and cost of the inspection.

Practice Limitation – (Non Disciplinary) – Physician is limited in that he shall not practice medicine in the State of Arizona and is prohibited from prescribing any form of treatment including prescription medications until he applies to the Board and receives its affirmative permission to do so.

Interim Practice Limitation- Respondent is prohibited from engaging in healthcare tasks in the STate of Arizona as set forth in A.R.S. § 32-2501(13) until he applies to the Executive Director and receives permission to do so.

Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until pendingof a formal hearing in this matter.

Surrender of License Effective 11-8-19

Prohibited from recommending, providing, prescribing, or issuing a written certificate for medical marijuana.

Interim Practice Restriction - Respondent is prohibited from prescribing controlled substances in the State of Arizona pending the outcome of a formal interview or formal hearing in this matter. Respondent may request, in writing, release and/or modification of this Interim Consent Agreement. The Executive Director, in consultation with and agreement of the lead Board member, and the Chief Medical Consultant, has the discretion to determine whether it is appropriate to release Respondent from the Interim Consent Agreement.

Interim Practice Limitation – (Non Disciplinary) – Physician is prohibited from engaging in direct patient care and shall be limited to no more than 10 working hours per week in the State of Arizona until he applies to the Executive Director and receives permission to do so.

Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until Respondent applies to the Executive Director and receives permission to do so.

Complete Revocation of license to practice Osteopathic Medicine in the State of Arizona.

Consent Agreement for Civil Penalty. The consent agreement includes a $250.00 civil penalty.

Consent Agreement and Order for Suspension and Probation. The consent agreement includes a six month suspension followed by probation for four years and six months.

Interim Practice Restriction – Respondent shall not prescribe controlled substances in the State of Arizona and shall not have access to controlled substances in the workplace during the period of this Order.
