Respondent’s license to practice allopathic medicine in the State of Arizona is summarily suspended. Respondent is prohibited from practicing medicine in the State of Arizona and is prohibited from prescribing any form of treatment including prescription medications or injections of any kind.

Practice Limitation – (Non Disciplinary) – Physician is limited in that he shall not practice medicine in the State of Arizona and is prohibited from prescribing any form of treatment including prescription medications until he applies to the Board and receives its affirmative permission to do so.

Interim Practice Restriction – Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until Respondent applies to the Executive Director and receives permission to do so.

Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-2501 (13) until he applies to the Board’s Executive Director and receives its affirmative permission to do so.Respondent may not apply for relief from this Practice Limitation until he has completed any current treatment program, has been found safe to perform health care tasks, and the Board is in receipt of any discharge report and recommendations for aftercare. This Order supersedes previous Interim Practice Limitation.
