Interim Practice Restriction – Respondent shall not prescribe controlled substances in the State of Arizona and shall not have access to controlled substances in the workplace during the period of this Order.

Interim Practice Limitation-(Non Disciplinary)-Physician is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-1401(22) until he applies to the Board and receives its affirmative permission to do.

Respondent is prohibited from engaging in the practice of medicine in the State of Arizona as set forth in A.R.S. § 32-2501 (13) until he applies to the Board’s Executive Director and receives its affirmative permission to do so.Respondent may not apply for relief from this Practice Limitation until he has completed any current treatment program, has been found safe to perform health care tasks, and the Board is in receipt of any discharge report and recommendations for aftercare. This Order supersedes previous Interim Practice Limitation.
