Apply For a License

Attention Pharmacists: Your license will be processed after we receive your final score from NABP and complete a substantive review. Please create an online profile to verify that a license number has been issued. You may print a copy of your license from your profile once you see a license number. 
Please do not reapply - there are no refunds.

 

 

To obtain an Arizona Fingerprint Clearance Card, you may apply online at https://psp.azdps.gov/services/fccFormTriage.

For questions about the process, you may contact the Department of Public Safety (DPS) by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.

 

FULL EXAM

Full Exam applicants will be taking both the Naplex exam and the Arizona MPJE exam for Arizona. It is not necessary to take these exams in Arizona.

To apply for an Arizona pharmacist license by full exam, you must have the following qualifications: 

  1. You must be a graduate of an accredited US university Pharmacist degree program.
  2. If you are a graduate of a foreign university, you must submit a copy of your FPGEC certificate. You must also have earned 1500 intern hours in the US.

SCORE TRANSFER

Score Transfer applicants will have selected to transfer the score of the Naplex exam taken in their home state to Arizona and will only take the Arizona MPJE exam. You must have selected Arizona as score transfer state at the time of Naplex registration or within 90 days of taking the exam. If you did not select Arizona as a score transfer state you must apply by reciprocity. 

To apply for an Arizona pharmacist license by score transfer, you must have the following qualifications: 

  1. You must be a recent graduate of an accredited US university Pharmacist degree program.
  2. If you are a graduate of a foreign university, you must submit a copy of your FPGEC certificate. You must also have earned 1500 intern hours in the US.

RECIPROCITY

This application is for Pharmacists who have a primary license earned by Naplex exam in another state. It is also for recent graduate Pharmacists who did not score transfer their Naplex score to Arizona within the 90 day transfer period.

 

To ensure a smooth application process, we recommend following the steps below in order.

1. Take and pass the Multi-State Pharmacy Jurisprudence Examination (MPJE)

2. Submit a license transfer application with NABP

3. Obtain AZ fingerprint clearance card from AZ DPS

4. Once steps 1-3 have been completed and you have received a clearance card in hand, you will then submit a pharmacist reciprocity application with us.

 

To apply for an Arizona pharmacist license by reciprocity, you must have the following qualifications:

  1. You must be a US citizen, permanent resident (green card), or have a valid US passport.
  2. The license to be reciprocated must have been obtained by examination (a primary license by Naplex exam) and must be open in good standing in your original state of licensure. If it is not current and in good standing at this time DO NOT APPLY FOR LICENSURE. Please contact the state where you hold your primary license and find out what needs to be done to bring it current.
  3. If you are a graduate of a foreign university, you must submit a copy of your FPGEC certificate. You must also have earned 1500 intern hours in the US.

Renew License 

•  Fingerprint clearance cards are not required for renewal


Pharmacists will need to log in through their profile to renew their license online. Creating a profile is free.

CE Requirements

Pharmacists may also submit a hard copy renewal form with a check or money order by postal mail or at the Board office.
Submitting a hard copy renewal form can take 3-4 weeks for processing.

The Arizona State Board of Pharmacy no longer provides hardcopy verifications of licensure, however we are still able to offer this service via our website.

Disciplinary Actions
