Technician License Information

To obtain an Arizona Fingerprint Clearance Card, you may apply online at https://psp.azdps.gov/services/fccFormTriage.

For questions about the process, you may contact the Department of Public Safety (DPS) by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.

 

 

PHARMACY TECHNICIAN TRAINEE

 
 

Please note: You may not apply for a Pharmacy Technician Trainee Registration if you previously held a Pharmacy Technician Trainee license.

 

Pharmacy Technician Trainee Registration applicants must submit :

  1. $25.00 application fee (this fee is non-refundable)
  2. Proof of legal United States residency and birth date
  3. Government-issued photo ID (If the document you submit to prove your legal residency does not contain a photograph).
  4. Documentation of any name changes.

PHARMACY TECHNICIAN (PTCB OR EXCPT)

Please note: This license is intended for individuals who have passed the PTCB or ExCPT examination and have never held a technician license with this Board. If you have not yet passed the PTCB or ExCPT examination, please refer to the technician trainee application. If you held a technician license with this Board, and that license has expired, please contact the Board office about renewing your license.

Pharmacy Technician applicants must submit :

  1. $82.00 License application fee (this fee is non-refundable)
  2. Proof of legal United States residency and birth date.
  3. Government-issued photo ID. (If the document you submit to prove your legal residency does not contain a photograph).
  4. Proof of high school graduation or its equivalent.
  5. Proof of passing the PTCB or ExCPT exam.
  6. A verification of licensure or certification, if applicable.
  7. A copy of a valid Arizona fingerprint clearance card.
  8. Documentation of any name changes.
  9. Court documentation, if applicable.
  10. Disciplinary orders, if applicable.
  11. License denial orders, if applicable.

NOTE: Reinstating a Pharmacy Technician license cannot be done online, click here for PDF form

   •  Fingerprint clearance cards are not required for renewal

    •  Technician Trainee licenses can not be renewed

Disciplinary Actions
