Due to the Health Emergency, pharmacy technician trainee licenses that expire 11/30/2020 will be extended to June 30, 2021.  The Board will re-evaluate again in June 2021 if additional extensions are required.  The extension will apply to pharmacy technician trainee licenses that expire while we are in a Health Emergency.  Please note:  This extension will continue to be evaluated periodically, and so it is highly encouraged to become nationally certified and apply for your pharmacy technician license.

To obtain an Arizona Fingerprint Clearance Card, Arizona residents may apply online at https://www.azdps.gov/services/public/fingerprint. 

Applicants who are not in Arizona must contact DPS for an application packet. You may contact DPS by calling 602.223.2279. DPS office hours are Monday through Friday, 8:00 am to 5:00 pm.

 

Pharmacy Technician Trainee

Pharmacy Technician Trainee applicants must submit :

  1. $60.00 license application fee (this fee is nonrefundable).
  2. Proof of legal residency and birth date.
  3. Government-issued photo ID.
  4. Proof of high school graduation or its equivalent.
  5. A verification of licensure or certification, if applicable.
  6. A copy of a valid Arizona fingerprint clearance card.
  7. Documentation of any name changes.
  8. Court documentation, if applicable.
  9. Disciplinary orders, if applicable.
  10. License denial orders, if applicable.

Pharmacy Technician (PTCB or excpt)

Pharmacy Technician applicants must submit :

  1. $82.00 License application fee (this fee is nonrefundable)
  2. Proof of legal residency and birth date.

  5. Proof of passing the PTCB or ExCPT exam.
  6. A verification of licensure or certification, if applicable.
  7. A copy of a valid Arizona fingerprint clearance card.

  10. Disciplinary orders, if applicable.
  11. License denial orders, if applicable.

Pursuant to A.R.S. § 32-3214(C), a person may obtain additional public records related to any licensee or certificate holder, including dismissed complaints and nondisciplinary actions and orders, by contacting the Board directly. Please click here to submit a public records request.


NOTE:  to expedite the application process as well as be more environmentally friendly, we ask that you please apply for any needed licenses online if at all possible (this does not apply to Technician Reinstatement requests or licenses or permits with expiration dates prior to 10/31/2019 – these cannot be done online). If you require a hardcopy version of any applications not listed below, please email our staff.

OCCUPATIONAL REGULATION
Pursuant to section 41-1093.01, Arizona Revised Statutes, an agency shall limit all occupational regulations to regulations that are demonstrated to be necessary to specifically fulfill a public health, safety or welfare concern. Pursuant to sections 41-1093.02 and 41-1093.03, Arizona Revised Statutes, you have the right to petition this agency to repeal or modify the occupational regulation or bring an action in a court of general jurisdiction to challenge the occupational regulation and to ensure compliance with section 41-1093.01, Arizona Revised Statutes.
