Intern License Information

To apply for an Arizona intern license you must have the following qualifications:
- You must be enrolled and seated in class in a US university accredited pharmacist degree program, or be a recent (less than 1 year) graduate of an accredited program.
- If you are a graduate of a foreign university, you must have an FPGEC certificate.
- You must also be a US citizen or a legal resident with permission to work in the United States and must meet all current US immigration policies. If you are here on a student F-1 visa or H-1B visa, please contact the Licensing Coordinator before submitting your application.

The Arizona State Board of Pharmacy no longer provides hardcopy verifications of licensure, however we are still able to offer this service via our website.

Pursuant to A.R.S. § 32-3214(C), a person may obtain additional public records related to any licensee or certificate holder, including dismissed complaints and nondisciplinary actions and orders, by contacting the Board directly. Please click here to submit a public records request.
NOTE: to expedite the application process as well as be more environmentally friendly, we ask that you please apply for any needed licenses online if at all possible (this does not apply to licenses with expiration dates prior to 10/31/2019 – these cannot be done online). If you require a hardcopy version of any applications not listed below, please email our staff.
